Office Information:

Applying for WIC in Louisiana

Eligibility:
    Residency: Must be a resident of Louisiana.
    Category: Pregnant, breastfeeding postpartum (up to 1 year), new mother (up to 6 months postpartum), infant, or child under 5 years old.
    Income: Meet income guidelines based on family size and income. Check the guidelines online.
    Nutritional Risk: Determined to be at nutritional risk by a healthcare professional (doctor, nurse, etc.).



Documents Required:

For either application method, be prepared to submit the following documents (originals and copies):

    Proof of residency: Driver's license, utility bill, etc.
    Proof of identity: ID card, birth certificate, etc.
    Proof of income: Pay stubs, W-2 form, etc. for all household members.
    Proof of pregnancy or breastfeeding: If applicable.
    Child's health record: If applicable.
    Healthcare professional's statement of nutritional risk: If applicable.

The Official WIC program is available to low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers can also bring their children to apply for WIC. You may apply for WIC if you are working or unemployed. Check the Income Guidelines Table to see if you are eligible for WIC.     I have a question I lived in New York and there I applied for the wic and they approved it, there will be some way that I can make the transfer to your office in Louisiana that I am here to stay. what diligence do I have to do to get it here in Louisiana I was also going to ask him if he could do it over the phone or in person
