Office Information:

Applying for WIC in Louisiana

Eligibility:
    Residency: Must be a resident of Louisiana.
    Category: Pregnant, breastfeeding postpartum (up to 1 year), new mother (up to 6 months postpartum), infant, or child under 5 years old.
    Income: Meet income guidelines based on family size and income. Check the guidelines online.
    Nutritional Risk: Determined to be at nutritional risk by a healthcare professional (doctor, nurse, etc.).



Documents Required:

For either application method, be prepared to submit the following documents (originals and copies):

    Proof of residency: Driver's license, utility bill, etc.
    Proof of identity: ID card, birth certificate, etc.
    Proof of income: Pay stubs, W-2 form, etc. for all household members.
    Proof of pregnancy or breastfeeding: If applicable.
    Child's health record: If applicable.
    Healthcare professional's statement of nutritional risk: If applicable.

Mission Statement: The mission of the Louisiana Department of Health is to protect and promote health and to ensure access to medical, preventive and rehabilitative services for all citizens of the State of Louisiana.

The Official WIC program is available to low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers can also bring their children to apply for WIC. You may apply for WIC if you are working or unemployed. Check the Income Guidelines Table to see if you are eligible for WIC. This website was created for women looking for WIC information and locations. We are also working towards adding other locations and services that may help out women.

    Arrived for wic appointment today for the first visit. Receptionist was nice But the staff member who approves your wic was very rude and inconsiderate. She did not inform me of wic benefits or educate me on how to use wic benefits. She rudely told me to read for myself. (her words reading is Fundamental) and proceed to walk off. Do not recommend this facility or staffing.

    I have called and left several message over the last two weeks to schedule a wic appointment. I have two grandchildren placed in my care by CPS and need assistance. No one has bothered to call me back.

    Good place for wic but they slow and it take them forever to return your call ! They never answer the phone
