Office Information: Bay CHD offers financial screening (PEPW), free immunizations and WIC. Clients choose one of several private doctors that will accept Medicaid for prenatal care.
For women who do not qualify for funding sources and are unable to get Medicaid (illegal alien), this program will attempt to make other arrangements on a case-by-case basis.    
Appointments generally last between one and two hours One-on-one nutrition counseling and assessment from a licensed nutritionist or qualified professional Information about nutrition and health to help you and your family develop healthy eat habits and lifestyles Breastfeeding support and information for you and your baby Checks for nutritious foods Referrals for other health care and community servicesWhat to Bring to a WIC Certification Appointment:    The child or baby that is being certified on WIC    Identification for everyone that has an appointment (ex. photo ID, baby\'s crib card, shot record, social security card, etc, )    Proof of income if you are NOT on Medicaid, Temporary Cash Assistance or Food Stamps (ex. paystubs, etc)    Proof of address (ex. Drivers license, utility bill, etc.)    Child\'s updated shot record    Your WIC card (if you are currently participating in WIC)


Mission Statement: To protect, promote & improve the health of all people in Florida through integrated state, county, & community efforts.

The Official WIC program is available to low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers can also bring their children to apply for WIC. You may apply for WIC if you are working or unemployed. Check the Income Guidelines Table to see if you are eligible for WIC. This website was created for women looking for WIC information and locations. We are also working towards adding other locations and services that may help out women.
