    For WIC services you no longer have an appointment and get babies weight or etc, you just pull in-front the doors in a carpool line and they bring everything out.

The Official WIC program is available to low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers can also bring their children to apply for WIC. You may apply for WIC if you are working or unemployed. Check the Income Guidelines Table to see if you are eligible for WIC. This website was created for women looking for WIC information and locations. We are also working towards adding other locations and services that may help out women.



Office Information: Office hours:
8:00 am to 4:30 pm Monday to Friday.


The Louisiana WIC Program provides enrolled participants with Breastfeeding education and support, nutrition counseling and education, nutritious food benefits, and referrals to medical providers and other programs.
Federal Regulations and state policy require that each applicant provide documented proof of identification, residency,household income, and pregnancy (when applicable) to determine eligibility for enrollment in the WIC Program.

Documents to bring:
1.
Person(s) Applying for WIC.
Bring each person who is applying for WIC to the clinic with you on the appointment date.
Bring an extra diaper for infants and children who are applying for WIC since they must be weighted with a clean, dry diaper.

2.
Immunization Records(s)Bring the immunization records for all infants and children applying for WIC, if available.

3.
Proof of Identification.
You should bring identification for yourself and for each person who is applying for WIC.



The following are acceptable proof of identification:
For Infants and Children, Birth Certificate or Birth Record, Nursery ID Bracelet, Valid Medicaid Card, Medical Record, Insurance Card, or Immunization records. For Adults, Birth Certificate or Medical Records (excluding immunization record), Current Drivers License or State Issued ID Card, Current Passport or Immigration Card, Valid Insurance Card or Medicaid Card, Valid Voter Registration Card

4.
Proof of Residency (For infants and children, proof must be provided for the parent/caretaker also.)You should bring proof of where you live.
The document should include your name, physical address, city, state, and zipcode.

The following are acceptable for proof of residency:,
Valid Medicaid Card, Valid Voter Registration Card, Current Rent or Mortgage receipt showing address of residence and name of renter, Current pay stub showing employee address, Current utility bill (gas, electric, water, phone, etc.), State or local document indicating Louisiana residence, i.e., property tax notice, Processed mail addressed to the participant or parent/caretaker at the physical address, For migrants or victims of theft, loss or disaster with no proof of residence, a statement written & signed by staff of a social service agency, church, legal aid society, school counselor, or nurse attesting to residence.

Mission Statement: The mission of the HIV/AIDS Surveillance Program is to Monitor the HIV/AIDS epidemic over time; Describe the changing characteristics of the epidemic; Define its impact on various subpopulations; Identify trends in modes of HIV transmission; Monitor behaviors associated with HIV transmission; Evaluate compliance with current USPHS recommendations for care and treatment; and Define mortality of HIV disease.
