The Official WIC program is available to low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers can also bring their children to apply for WIC. You may apply for WIC if you are working or unemployed. Check the Income Guidelines Table to see if you are eligible for WIC. This website was created for women looking for WIC information and locations. We are also working towards adding other locations and services that may help out women.

Proof of Identity for the Person/Child Being Enrolled (Bring ONE)
- Birth Certificate
- Drivers License or MVA Identification Card
- Social Security Card
- Picture Identification (Government, School, Employment)
Proof of Address (Bring ONE)
- Drivers License
- Official Bill or Statement
- Copy of Lease
Proof of Household Income (Bring AT LEAST ONE)
- Recent Pay Subs or Benefits Letter: SSI, Social Security, Unemployment
- Medical Assistance Card
- Independence Card with Recent Receipt (30 days)
- Tax Return
Proof of Pregnancy (If Pregnant)
Immunization Record (Infant or Child)
WIC Referral Form (If Available)
If you are already enrolled in WIC, bring your WIC ID Folder to ALL appointments including check pick-up
The person to be enrolled MUST come to the appointment.
