Office Information: The WIC programs complete name is Special Supplemental Nutrition Program for Women, Infants and Children. WIC helps lower-income women (pregnant, breastfeeding, and those who recently had a baby) and infants and children (up to age 5) who are at nutritional health risk.

WIC Clinic Hours at CCHD:
Mondays: 8am 11am, 1pm 4pm
Tuesdays: 8am -11am, 1pm 4pm
Wednesdays: 8am-11am, 1pm-4pm
Thursdays: 8:30am 4:30pm Walk- In Day/No Appointment Necessary
Fridays: 8am -11am, 1pm 4pm

Proof of pregnancy is not required.

Bring the following applicable items with you:

Proof of Identity (examples)

Photo ID
Drivers License
School ID
Birth Certificate
MT Medicaid card/HMK Plus
Passport
Tribal ID
Social Security card



Proof of Identity for Child (examples)

Shot/immunization records
Birth certificate/record
Foster care/adoption papers
Healthy Montana Kids Plus card
Social Security card



Proof of Residency (examples)

Mortgage statement
Utility bill (power, water, cable)
Rent receipt
Note from landlord
Lease/housing agreement



Proof of Household Income for all people living in your house (examples)

One month of most current documented income (paystubs)
Proof of adjunctive eligibility (SNAP, HMK Plus, Medicaid, TANF, FDPIR, School Lunch)
Recent 1040 tax statement (only if you own your own business)
Additional income information (Child Support, Unemployment, etc.)









The Official WIC program is available to low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers can also bring their children to apply for WIC. You may apply for WIC if you are working or unemployed. Check the Income Guidelines Table to see if you are eligible for WIC. This website was created for women looking for WIC information and locations. We are also working towards adding other locations and services that may help out women.
