Call the WIC office at 518-288-3232, they ask a few questions and if it sounds like you might be eligible for WIC, they will schedule an appointment for you.
Saratoga County EOC, Inc is a local WIC clinic in New York.
To apply for WIC at this office you must qualify.
1. A pregnant woman.
2. An infant or child up to 5 years old.
3. A mother of a baby that is up to six months old or a breastfeeding mother of a baby that is no older than 12 months old.
Other rules include:

1. Be a resident of New York State (you do not need to be a US citizen).
2. You must also meet specific income eligibility requirements (visit the State of NY Department of Health website at http://www.health.state.ny.us for rates) or be receiving Medicaid or Food Stamps.
3. Your nutritional and medical needs must be assessed by WIC health care professional to find out which benefits apply.
Height, weight and a simple blood test for hemoglobin will be taken if current medical information is not available or provided. (Hemoglobin is required for children over 9 months of age and prenatal, post-partum and breastfeeding women applying for WIC).
Proof of all household income Income and documents to bring:

Paycheck stubs they need the most recent 4 weeks for everyone working in your household (if you have direct deposit make sure you pick up your stubs for your WIC appointment. LES (military most recent full month) Proof of unemployment benefits
Proof of child support / alimony Self-employed only may use prior year income tax form Medicaid card(s) for everyone on Medicaid Food Stamps bring the current Food Stamp Action Letter listing names on the Food Stamp case and eligibility period Foster care payments Proof of Identification for each applicant (ex., drivers license, Social Security card, birth certificate) Proof of residency recent postmarked envelope, lease agreement, mortgage document, your utility bill, cable bill (no PO Boxes) Immunization records for each child applying for WIC Proof of pregnancy If you are pregnant you need to bring proof of pregnancy




The Official WIC program is available to low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers can also bring their children to apply for WIC. You may apply for WIC if you are working or unemployed. Check the Income Guidelines Table to see if you are eligible for WIC.
